Our online reservation page offers two registration paths depending on your situation. Please follow the steps below.
STEP 1. Access the reservation page
Visit the Online Reservation Page.
STEP 2. Open “マイページ” (My Page)
Select “マイページ” (My Page) from the menu.
STEP 3. Choose your registration path
Scroll to the bottom of My Page and select the option that matches your situation.
Pattern A — First time visiting the clinic
Select “※当院のご利用がはじめての方はこちらです。” (For those who do not yet have a patient card.)
Pattern B — You have a patient card, but new to online booking
Select “※診察券をお持ちでネットからの予約がはじめての方はこちら” (For existing patients who already hold a patient card number.)
STEP 4. Create your account
Pattern A fields
- Login ID
- Password / Password confirmation
- Last name & first name (kanji)
- Last name & first name (katakana)
- Date of birth
- Gender
- Email address
- Phone number
Pattern B fields
- Login ID
- Password / Password confirmation
- Patient card number
- Email address
- Phone number
- Date of birth
*On subsequent visits, only your Login ID and Password are required.*
STEP 5. Select department & visit type
Choose your specialty and whether this is an initial or follow-up visit.
- Psychosomatic Medicine / Psychiatry
- Internal Medicine
- Telemedicine
STEP 6. Select date and time
Check availability on the calendar and select your preferred slot.
STEP 7. Confirm your reservation
Review your entries and press “Confirm Reservation”. A confirmation email will be sent to your registered email address.
Change or cancel
You can change or cancel your reservation anytime from My Page.
