About the Welfare Medical Subsidy Program

Keldsen Family Clinic is a designated medical institution under the Welfare Medical Subsidy Program.

Overview

Treatment for conditions in the fields of Psychosomatic Medicine and Psychiatry often requires regular, ongoing outpatient care, which can lead to significant medical expenses. To help ease this financial burden and make it easier to access care, a public assistance program exists that covers a portion of outpatient medical costs — including consultations, tests, and medications — under the Welfare Medical Subsidy Program (Mental Health Outpatient Care). With this program, your out-of-pocket cost is just 10%, regardless of your insurance type. (A monthly cap on total out-of-pocket expenses also applies, based on income.)

How to Apply

Certain conditions must be met to receive benefits, and a Medical Certificate from your doctor is required. If you are interested, please speak with your doctor or our front desk. We will prepare the Medical Certificate, and you will then gather the remaining required documents and complete the application at your local municipal office. Please note that you will need to designate a specific medical institution and pharmacy when applying — the 10% co-pay rate applies only at those designated locations.

After Your Application

Your 10% co-pay rate takes effect from the date your application is submitted. After that, please present your Welfare Medical Subsidy Certificate at the front desk at each visit. The certificate is valid for one year, so please remember to renew it annually if needed. (A Medical Certificate from your doctor is required once every two years for renewal.) If you have any questions, please don't hesitate to ask our front desk staff.

About the Mental Disability Welfare Certificate Program

The Mental Disability Welfare Certificate is a program designed to support the independence and social participation of individuals who continue to experience significant limitations in daily or social life six months or more after their first medical consultation. Certificates are issued in three grades based on the degree of impact on daily and social functioning: Grade 1 — the most severe disabilities, typically requiring constant care; Grade 2 — requiring substantial support in daily life; Grade 3 — some limitations in daily life or social activities, but with a degree of independent functioning possible. The benefits and discounts available may vary by grade. Certificate holders may be eligible for discounts on public utilities, tax exemptions and reductions, employment under disability hiring quotas, and priority access to public housing, among other supports. At Keldsen Family Clinic, we issue Medical Certificates for eligible patients applying for the Mental Disability Welfare Certificate. Please feel free to speak with us.

About the Disability Pension Program

The Disability Pension Program is a pension benefit program for individuals whose daily life or ability to work has been significantly affected by a psychiatric condition. Its purpose is to provide financial support as part of a broader social safety net for those living with persistent mental health disabilities. The program is available to individuals recognized as having difficulty with daily life or employment due to a psychiatric condition. Eligible conditions may include Schizophrenia, Depression, Bipolar Disorder, developmental disorders, and others.

To be eligible for benefits, the following conditions must be met:

  • You meet the eligibility criteria for disability pension (e.g., your first consultation date falls within your pension enrollment period, and at least 1 year and 6 months have passed since your first consultation for the qualifying psychiatric condition)
  • The severity of your disability meets the criteria established under the pension program
  • A Medical Certificate from your attending physician and your medical institution, as well as documents describing your daily living situation, will be required

The Disability Pension Program uses a grading system based on the severity of disability, and the amount of pension paid varies by grade. In general, recipients are classified into Disability Grade 1, Disability Grade 2, or Disability Grade 3.

  • The Disability Pension Program includes two types: the Basic Disability Pension and the Employee Disability Pension.
  • Basic Disability Pension (Grades 1 and 2 only): Available to those enrolled in the National Pension, such as self-employed individuals and students.
  • Employee Disability Pension (Grades 1 through 3): Available to those enrolled in the Employees' Pension Insurance, such as company employees and public servants.

Documents Related to Employment

  • Certificate for Leave of Absence

    A medical certificate in which the physician confirms that the patient needs to take a leave of absence from work (or school) in order to focus on their recovery.

  • Return-to-Work Certificate

    A medical certificate in which the physician confirms that a patient who has been on leave of absence is now fit to return to work.

  • Sick Pay Allowance Application Form

    The Sick Pay Allowance is a benefit provided by health insurance when illness makes it difficult to work. Your attending physician will complete the required fields in the designated section of the application form.

  • Attending Physician's Opinion Letter / Illness & Injury Certificate (for Hello Work)

    When registering for job-seeking services or handling unemployment insurance procedures at Hello Work, you may be required to submit an Attending Physician's Opinion Letter or an Illness & Injury Certificate. Below is an overview of each document and how to obtain them.

    The Attending Physician's Opinion Letter is a document in which the physician evaluates and records the stability of symptoms and the patient's ability to work, for individuals with a mental disability registering for job-seeking services at Hello Work. This allows Hello Work to provide appropriate job placement support.

    The Illness & Injury Certificate is a document in which the physician certifies that a job seeker is unable to work due to illness or injury. It is used when applying for unemployment insurance sick benefits or when justifying a temporary suspension of job-seeking activities.

    Individuals enrolled in unemployment insurance who become unemployed and register for job-seeking services at Hello Work may be eligible to receive basic unemployment benefits. If you are unable to work due to illness or other reasons, submitting an Attending Physician's Opinion Letter or Illness & Injury Certificate to Hello Work may allow you to receive benefits for up to three years.

  • Work Capability Certificate (for Hello Work)

    A document in which the physician certifies that a job seeker is fit to work. This certificate may be required during unemployment insurance benefit procedures or when applying for jobs.

    Unemployment Insurance Benefits: This certificate is submitted to confirm that you are fit to work when resuming job-seeking activities after having extended your unemployment insurance benefit period due to illness or injury.

    Job Applications: Some employers may request this certificate as proof of your health status when you apply for a position.
